Admission is open to patients of any age, racial or religious background who:

• Are a Singapore Citizen or Permanent Resident
• Have a doctor’s referral. Please click here for referral form.
• Are diagnosed to have an advanced progressive illness with a prognosis of less than 12 months
• Have a caregiver at home who can tend to their activities of daily living
• Are in residence or going to be discharged from a hospital

Our home care services are free. However, patients need to purchase their own medication.
