Metta Day Rehabilitation Centre for the Elderly

Metta Day Rehabilitation Centre for the Elderly (慈光乐龄日间康复中心) was established in June 1998 to provide rehabilitative care to those suffering from physical impairment due to stroke, Parkinson’s disease or other injuries.

Our services are geared towards maximising our patients’ independence level, while offering respite to their families and caregivers regardless of race, language, or religion. Our centre executes our vision of care to maximise their functional level by providing a comprehensive range of rehabilitative and social support services all under one roof.

We focus on assisting our daily clientele in regaining maximum functional capability through rehabilitative treatment and engaging in active lifestyles by means of social programmes.

Rehabilitation

Both active and maintenance rehabilitation are conducted for recipients who require follow up care after discharge from hospitals or affected by age related frailty and conditions such as stroke and arthritis.

Our core services include physiotherapy, occupation therapy and nursing assessments.

We also support our care recipients’ families to become more adept in their caregiving roles by loaning them mobility equipment such as walking aids or wheelchairs and conducting regular caregiver training courses to equip them with basic nursing skills.

Social Programmes

Ensuring continuity between step down care and community based eldercare are social programmes designed to provide supervision and supportive help for care recipients who have progressed from rehabilitative to maintenance programmes.

Some of the activities we conduct include art, gardening and taichi. Outings to various places of interest and activities are organised in collaboration with community partners to engage our patients socially and improve their quality of life.

Severe Disability Assessment (SDA)

Metta Day Rehabilitation Centre for the Elderly (Metta) is a MOH-accredited disability assessor which includes trained therapists and nurses to conduct the severe disability assessment.

Persons with disabilities who wish to apply for financial assistance schemes such as CareShield and ElderShield will require to submit this assessment report to determine their eligibility.

Our assessor will be able to conduct the assessment either at our centre or at your home and they will submit the assessment report directly to AIC. Please contact us at 6789 5951 or email us at rehab@metta.org.sg for an appointment.

The assessment evaluates one’s capacity to perform 6 activities of daily living (ADLs) unassisted.

Value Added Programmes

A balance-and-fall programme aimed at preventing and reducing falls among the elders serves to provide neuromuscular and vestibular training to strengthen their lower limb muscles and improve balancing ability.

Additionally, counselling services are available to render psychosocial support for individuals, couples and families from various socio-economic backgrounds.

Admission

Admission is open to individuals of any age, race or religion who:

  • Singapore Citizen or Permanent Resident
  • Suffer from medical conditions that require physical rehabilitation
  • Sufficiently stable to actively participate in Rehab programmes
  • Referred by Agency for Integrated Care (AIC)

Metta Hospice Care

Metta Hospice Care (慈光安宁居家护理中心) is a member of Singapore Hospice Council and Asia Pacific Hospice Network. Set up in June 2000 to offer free home hospice care to the terminally ill of all ages and races, we have since helped more than 5000 patients to live with quality and dignity throughout the last phase of their lives at the comfort of their homes.

We primarily take care of patients who are referred from medical institutions and general practitioners. They are diagnosed with life-limiting illnesses such as cancer and end stage renal failure and usually have a prognosis of less than a year.

We provide a holistic care approach with medical consultations and nursing assessments to our care recipients, as well as psychosocial support to them and their families. Our Social Worker will also support our care recipients and their families on the emotional and social aspects if required.

Palliative and Hospice Care

Palliative and hospice care form the two main tenets of our services. While the former entails pain relief and management of other symptoms such as nausea and fatigue, the latter tends to the physical, emotional, psychological and spiritual needs of patients and their caregivers.

They represent a holistic approach that aims to alleviate suffering and maximise quality of life. Aligned to this goal is our comprehensive range of medical, nursing, social, emotional and spiritual support which ensures our patients undergo a smooth transition from acute care in hospitals to a home care environment that is clean and safe.

To facilitate the development of individualised care plans, we work with doctors to first assess their needs and conditions in order to determine the most suitable type of care. The intensity of home visits and reliance on medical equipment are then paced according to the severity of their conditions during subsequent visits.

Besides administering medication and counselling, our medical personnel work with the patients’ primary physicians to achieve total care management. We also loan medical equipment such as hospital beds, oxygen concentrators, wheelchairs, walking aids and commodes free of charge to facilitate care of our clients in the comfort of their homes.

Support Services

Believing that emotional and spiritual pain are as veritable as physical afflictions, we offer counselling services and spiritual support to both the patients and their families to help them cope with end-of-life issues, including grief.

At the same time, spiritual advisors from various faiths are available to help them connect to their religions or draw strength from personal values.

Additional care services as well as assistance in subsidy applications can also be arranged with medical institutions and social assistance providers.

Admission and Fees

Admission is open to patients of any age, racial or religious background who:

  • Singapore Citizen or Permanent Resident
  • Diagnosed to have an advanced progressive illness with a prognosis of less than 12 months
  • Referred by a doctor or Agency for Integrated Care (AIC)
  • Our home care services are free. However, patients need to purchase their own medication.

As we provide free home hospice care, we rely heavily on donations to defray our annual operating cost of about $500,000.

Metta HomeCare

Metta HomeCare (慈光居家护理服务) started in April 2015 as a pilot programme with Eastern Health Alliance to support the medical and nursing component of care in the community. It is a home-based service that provides a holistic approach of managing patients with chronic illnesses and organ failure.

Metta HomeCare provides a holistic approach to managing patients with chronic illnesses and organ failure. Our goal is to render early intervention and medical assistance with an individualised care plan to reduce the frequency of re-admission into the hospital. This allows our elderly to age in the comfort of their homes, regardless of race and religion.

After receiving the subvention grant from the Ministry of Health (MOH) in December 2016, Metta HomeCare started receiving referrals from hospitals as of 1 March 2017 and reached out to more beneficiaries in the eastern part of Singapore regardless of race or religion.

Metta HomeCare is committed to render free services to care recipients whose National Means Test is 60% and above to defray their expenses of aging in place.

Apart from medical consultations and nursing procedures, we also equip caregivers with basic nursing care skills to empower them to care for their loved ones. Medical equipment such as hospital beds, oxygen concentrators, wheelchairs, walking aids and commodes are given or loaned to our needy beneficiaries free of charge as it boosts the caregivers’ confidence in caring for their loved ones at home.

Metta HomeCare encompasses 2 key services – Metta Home Medical and Metta Home Nursing. Home Nursing visits are done by our trained nurses, registered with Singapore Nursing Board, to provide basic wound care, change of nasogastric tube, change of urinary catheters, medication packing, and regular nursing assessment.

Home Medical visits are done by doctors, registered with Singapore Medical Council, once a month or as and when necessary depending on their medical needs, in the comfort of their home.

Support Services

We have a Social Worker who will assist our care recipients and their families on the emotional and social aspects if required.

Additional care services and assistance in subsidy applications can also be arranged with medical institutions and other external social assistance providers.

We support our care recipients’ families and help them adapt to their caregiving roles by loaning them medical equipment like oxygen concentrators, wheelchairs and hospital beds. We also conduct basic caregiver training in order to equip them with the necessary nursing skills.

Last but not least, we strongly believe in extending beyond the mere provision of medical service to provide holistic social programmes for our patients. Some of the initiatives we have done include birthday celebrations, house cleaning and outings.

  • In residence or going to be discharged from a hospital
  • Referred by a doctor or Agency for Integrated Care (AIC)

Our home care services are free. However, patients need to purchase their own medication.

As MOH-funded community care provider, our clients receive subsidised services according to household means-test results.
